Judith Young, who was known as Judith Henry when she grew up in Orange, was last seen by her husband when she left her home at Gunningbland, near Parkes in mid-December 1999.
Police say they have grave concerns for the welfare of the missing woman.
In 2001, police set up Operation Ullswater, in a bid to find the 48-year-old woman.
Judith Young was a band manager and well known in music circles in the area.
When Operation Ullswater was first set up inquiries for the missing woman extended nation-wide and initial inquiries indicated the missing woman may have relocated to Queensland.
However, since then, police have been unable to verify that information.
Detectives from the Lachlan Local Area Command are urging anyone who may have seen or heard from Mrs Young since her disappearance, no matter how insignificant they believe the information to be, to come forward.
