Emergency services were called to a road near Forster about 1.30pm on Monday, following reports a car had hit a fence and flipped.
The driver and two passengers, a 46-year-old woman and five-month-old boy, were seriously injured.
The driver, a 27-year-old woman, was thrown from the vehicle and injured her spine, while the other 46-year-old woman has head and chest injuries, a spokesperson for the Westpac Rescue Helicopter Service said.
All three were treated at the scene by paramedics, before being airlifted to John Hunter Hospital in Newcastle.
The women remain in a serious condition, while the baby is in a critical condition with head injuries.
