The federal government invited the public to complete the survey, which will have a bearing on the final terms of reference.
LiveBetter chief customer officer Chris Rawlins said the royal commissions into banking and aged care had proved the format could expose practices which otherwise escaped attention.
"The hope for this royal commission is to ensure people with disability are protected, safe and treated with dignity and respect," he said.
"Now's the time for people to speak up."
