Orange teenager John Keegan was killed in a truck and car crash at the intersection on Friday night, which Mr Donato initially flagged as dangerous to Minister for Regional Transport and Roads Paul Toole in August last year.
Mr Donato has since written to Mr Toole three more times, once in September 2020 and twice more in February to request investigations on behalf of concerned community members, all of whom cited previous crashes and near misses.
Both Mr Toole and Transport for NSW confirmed the correspondence sparked an investigation which, although showing just one reported crash in the five years to 2020, resulted in additional signage being installed in January and April. Line markings were also improved.
Mr Donato, however, said the additional signage simply "isn't good enough" and called for more urgent and extensive action to be taken, some of which is now in Transport for NSW's plans.
"If they think putting up a few signs is going to do the job, well, it's just not the case. They need to do something now before more lives are lost there," Mr Donato said.
"It gets foggy and misty there and the signs could easily be missed. It's a notoriously dangerous intersection ... the roads needs widening and a slip lane to avoid vehicles turning right, it may even need a speed reduction."
Transport for NSW's response to Mr Donato's initial correspondence last year stated no road widening was planned for the area, however it's been confirmed that upgrade is now expected to be completed this financial year.
Isolated road widening to allow passage on the left, that slip lane Mr Donato is calling for, is among the additional measures expected to be delivered as a result of further community consultation sparked by the concerns.
"In response to the concerns raised, Transport for NSW has taken a number of actions ... including meeting with the community on site (in March) to discuss additional safety measures which are planned for the coming months," Mr Toole said.
"I've also asked the Centre of Road Safety to independently review the work planned and we will take its expert advice on any recommendations to make sure this intersection is as safe as possible."
Transport for NSW also confirmed it is amid a fatal crash investigation following Friday's tragedy.
"This is a tragic set of circumstances and my heart goes out to all involved," Mr Toole said.
