John Richard Szulik, of Shannon Street, Molong, was also ordered to pay $10,000 costs in a ruling handed down by the NSW Civil and Administrative Tribunal last week.
It found Szulik, then a registered nurse at Bloomfield Hospital, had gained inappropriate access to clinical records and engaged in a personal and intimate relationship with the female patient, known as Patient A, who had been seeking treatment for an eating disorder.
The hearing was told in papers lodged by the Health Care Complaints Commission Szulik provided nursing care to Patient A, then aged 16-17, during the first four of five times she was admitted to the Children's and Adolescent Mental Health Service Unit of the hospital in 2014.
The tribunal was also told he frequently asked Patient A about her weight when he knew it was inappropriate.
The HCCC alleged Szulik had talked to Patient A about his own sexual activities and asked her to disclose her sexual activities to him.
It also said Szulik contacted Patient A’s mother at her workplace, gave his own personal details and asked for the patient to contact him.
The tribunal was also told he frequently asked Patient A about her weight when he knew it was inappropriate.
“[He] ‘inappropriately engaged in flirtatious, casual or intimate communication by email with Patient A including stating 'What do you weigh? If you don't tell me you have to send the nudes!!’,” it heard.
“[And] ‘Tell me what you weigh...It's my birthday!! I may have to ring [Patient A's place of work] and say [Patient A] was very rude to me...Loll.”
The tribunal was told Szulik inappropriately attended Patient A’s home at least 12 times and on one occasion brought his two-year-old son along.
It was also told Szulik “inappropriately stole 'Resource' nutritional supplements from the hospital to bring to Patient A at her family home on at least one occasion.”
It heard Szulik did not renew his registration in 2015.
