While the Carcoar native has won marathons across the globe, Boston is one title he is yet to add to his impressive resume.
He placed third last year and has twice been runner-up, while on Monday he crossed the line in fourth spot, two minutes and 24 seconds behind victor Marcel Hug.
Fearnley lost time in the early downhill sections and trailed Hug by 97 seconds at the halfway mark of the 42.2 kilometre push. He could not make up that gap as the Swiss master was clocked a course record 1:18.04.
"It's always a tough start here for me as one of the lighter athletes and I just couldn't do enough to stick with Marcel in the first five kilometres,” Fearnley said. "I hate missing a podium, but am happy with my push. I managed to pull about 30 seconds back on the leaders in the middle section of the race."
