Police and emergency services were called to Razorback Road, Running Stream, following reports of a head-on crash.
Police said initial inquiries indicated a Range Rover was travelling eastbound when it collided head-on with a Mazda 6 wagon travelling westbound.
NSW Ambulance Paramedics treated a total of 10 people at the scene – six from the Range Rover and four from the Mazda.
A female passenger in the Range Rover, believed to be aged in her 60s, died at the scene.
Four people were airlifted to Orange Base Hospital in serious conditions and the other five people were taken by road to Orange Base Hospital.
The drivers will also undergo mandatory testing.
Officers from Mudgee Local Area Command attended and established a crime scene.
The road is expected to be closed for a number of hours while specialist investigators from the Crash Investigation Unit conduct inquiries.
